4-Fluoroethylphenidate (4F-EPH) is a synthetic stimulant in the phenidate family. It is the 4-fluorinated analogue of ethylphenidate, with a fluorine atom at the para position of the phenyl ring. The compound appeared on European research-chemical markets around 2015–2016. Until recently, its pharmacology was largely unknown; a 2026 rodent study has now reported stimulant-like, rewarding and reinforcing effects, while an earlier forensic paper described a death in which 4F-EPH was considered a possible contributor alongside other stimulants.

Chemistry and identification

4F-EPH is an ethyl ester of a piperidine-based phenylacetic acid derivative. It differs from 4F-MPH by having an ethyl rather than methyl ester, and from ethylphenidate by carrying a fluorine atom on the aromatic ring. It is a chiral compound.

The abbreviations 4F-EPH, 4-F-EPH and 4F-TEP have all been used in analytical and monitoring records. It should not be confused with 4F-MPH, which is a different compound with its own clinical and forensic literature.

Pharmacology

Until recently, there was little direct experimental information about 4F-EPH. Earlier reviews inferred that it might share the dopamine- and norepinephrine-reuptake inhibition of ethylphenidate, but stressed that this was an extrapolation rather than a demonstrated receptor or transporter profile (Corkery and Schifano, 2022).

A 2026 study tested 4F-EPH in rodents. It fully substituted for methamphetamine and cocaine in drug-discrimination experiments, produced conditioned place preference, increased locomotor activity and supported self-administration in mice. The authors interpreted these findings as evidence of rewarding and reinforcing properties and a high potential for abuse. These are preclinical results, not measurements of human potency or clinical effects (Kim and colleagues, 2026).

Reported effects

The available human descriptions come mainly from online reports collected in the 2022 review. They mention stimulation, increased energy, confidence and euphoria, although some users described a less pronounced effect than with ethylphenidate. These anecdotes do not establish a consistent pattern of subjective effects.

There are no controlled human studies defining the effects of 4F-EPH. Claims about focus, mood or duration therefore remain uncertain, particularly because the compound’s human pharmacokinetics remain poorly characterised.

Clinical and forensic findings

A 2022 paper described the first reported death involving 4F-EPH. The individual had also been exposed to MDMA and MDA, and the pathologist concluded that the death resulted from complications of psychostimulant use, particularly MDMA/MDA and potentially 4F-EPH. The authors were careful not to attribute the outcome to 4F-EPH alone (Corkery and Schifano, 2022).

The same review noted the absence of documented non-fatal hospital presentations specifically attributed to 4F-EPH at the time of publication. Its authors emphasised that routine toxicology screens may not detect phenidate analogues unless specific analytical methods are used.

Toxicity and research limitations

The 2026 rodent study adds evidence of abuse-related behavioural effects, but it does not establish the human toxic range or the likelihood of particular adverse reactions. Human pharmacokinetic data, dependence studies and long-term safety research remain limited.

The reported fatality involved multiple stimulants, so the contribution of 4F-EPH remains qualified. It is more accurate to describe the compound as a potential contributor in a mixed-drug death than to present it as a confirmed sole cause.

Related compounds

4F-EPH is related to Ethylphenidate, 4F-MPH and 3,4-CTMP. It is the para-fluoro analogue of ethylphenidate and the ethyl ester counterpart of 4F-MPH.
